TEAM MEMBERS: Crew of Three: PIC, Nurse, Paramedic
PATIENT ON BOARD: NO
ACCIDENT DESCRIPTION: Flight crew was in preparation to launch on a mission
when a hospital security officer walked underneath the tail boom while
aircraft was running. The security officer was struck in the head by the
tail rotor resulting in a fatal wound.
ADDITIONAL INFORMATION: Helipad was illuminated. Tail boom and tail rotor
were illuminated with an aircraft mounted light.
