DESCRIPTION	
	On approach to the landing zone while responding to a patient
	transport request, the aircraft crashed.  The cause of the crash is
	under investigation.

ADDITIONAL INFO	
	Air Evac Services, Southwest Ambulance and the emergency medical
	services community will be holding a memorial service to honor Renee
	Johnson on Tuesday, December 21, 2004 at The Mountain View Memorial
	Gardens at 10:00 a.m.  The Memorial Gardens are located at 7900 East
	Apache Trail, Mesa, AZ  85207.  Renee's internment will be held in
	Montana with her family.
