[Date Prev][Date Next][Date Index] AIRLIFE Denver - Non-injury incidentDate: 11/16/2006 1100 MST Program: AIRLIFE Denver 501 E. Hampden Avenue Englewood, CO 80113 Type: Lear Jet 25 Tail #: 600GM Operator/Vendor: International Jet Aviation Weather: Clear. Not a factor Team: 2 Pilots, 2 Neonatal Nurse Practitioners, 1Flight RT. No injuries reported. No patient. Description: While enroute to pick up a neonatal patient, when retarding the throttles during descent to the airport in Rapid City, SD, the # 1 engine flamed out. The engine was restarted according to procedure but flamed out again when throttles were brought to idle for the landing. The nurse crew continued on to the hospital to care for the patient and a second jet was dispatched to complete the mission. The original aircraft was inspected, repaired and flown back to base airport within 3 hours. Additional Info: After a thorough aircraft inspection and review of recent maintenance actions, the problem, in all probability, was a misadjusted throttle cable which occurred during a routine battery replacement. The Lear Jet throttle cables pass in close proximity to the aircraft battery. Maintenance procedures have been changed to include verification of proper throttle operation after all battery replacements and maintenance training has been enhanced to include emphasis on the potential for throttle cable maladjustment during battery changes.
