[Date Prev][Date Next][Date Index] Angel One - Non-injury incidentDate: 9/28/07 0200 CDT Program: Angel One Arkansas Children's Hospital 800 Marshall St. Little Rock, AR 72202 Type: S-76 C Tail #: N760AF Operator/Vendor: Own Part 135 Weather: Clear. Not a factor Team: Pilot, Flight Nurse, Flight Respiratory Therapist. No injuries reported. Patient on board. Description: At 0200, aircraft N760AF (Sikorsky S-76 C+) was transporting a patient from Baxter Regional Medical Center back to Arkansas Children's Hospital. While departing the hospital helipad with a patient on board, the aircraft experienced a power fluctuation and an unusual vibration. The crew took immediate action and landed the aircraft at the Baxter Regional airport. No injuries or damage were sustained to the crew or aircraft. N761AF was dispatched to transport the crew and patient back to ACH without further incident. Further investigation by maintenance staff in conjunction with Sikorsky engineering revealed a problem with the collective position indicator for the number 1 engine. While the faulty collective position indicator was running sluggish and caused the low side RPM problem, it was not giving enough of a Delta factor to cause a computer failure light. The collective position indicator was replaced, engines inspected and aircraft returned to service October 1, 2007.
