Type: AS 350 Tail #: N586AE Team: Pilot, flight nurse and flight paramedic. Injuries. Description: At approximately 0340 on Friday, June 27th, Air Evac 31 was involved in an accident while responding to a patient transport request. Air Evac 31 is based at Love Field in Prescott, Arizona. On approach to the landing zone in an area approximately 30 miles to the northwest of Prescott, the aircraft was involved in an accident. There was no patient on board the aircraft. The cause of the crash is under investigation by PHI, the FAA and the NTSB. Two of the flight crew members were transported by fellow air medical providers to Flagstaff Medical Center and are listed in serious conditions. The third flight crew member was transported by ground ambulance and has been released from the hospital.
