[Date Prev][Date Next][Date Index] Midwest Medflight - Non-injury incidentDate: 12/11/2008 05:00am est Program: Midwest Medflight 5305 McAuley Drive #311 Ypsilanti, MI 48197 Type: BK 117 Tail #: 8765J Operator/Vendor: PHI, Inc. Weather: Clear. Not a factor Team: Pilot, flight nurse, and flight paramedic. No injuries reported. No patient. Description: While on final approach for an interfacility transport, the Sunnex Series 740 high intensity halogen light assembly failed (high intensity light for patient care), which caused internal overheating. This in turn melted the internal portions of the light assemply causing a large amount of smoke in the aircraft. The flight nurse in the back of the aircraft notified the pilot who landed the aircraft safely at the intended destination. The aircraft was put out of service to be inspected by maintenance and the crew transported the patient by ground. Additional Info: The light assembly was removed by maintenance and the aircraft was put back into service within one hour and was flown back to base.
